SUBJECT:        Wheelchair Division State Cross Country Championships

2015 marks the 20th year that there will be a wheelchair division at the WIAA/Dairy Farmers of Washington/Les Schwab Tires Cross Country State Championships.  The State Cross Country meet management and the City of Pasco have done everything possible to ensure a safe and successful race for the wheelchair division.  To this end it is imperative that each school sending a wheelchair competitor take the time to make sure that each athlete is eligible under both WIAA and school rules.  All athletes are required to demonstrate compliance with those rules of eligibility in Article 18 of the WIAA Handbook and any corresponding school rules and regulations.

To assist in this process, please complete the bottom portion of this sheet and fax to Brian Smith at (425) 687-9476 no later than Friday, October 16, 2015.

I affirm that the athlete listed below meets all current WIAA and school requirements and is eligible to compete in the WIAA State Cross Country Championships.
